This can help a patient\u2019s doctor manage their care.\u00a0 In the past, health providers shared health records by telephone, fax, mail or limited computer networks.\u00a0 It was time-consuming and not the most reliable.\u00a0 Now doctors can locate a patient\u2019s health records in a matter of minutes with PHIX.\u00a0 The better flow of information between doctors, labs, and pharmacists gives a patient\u2019s doctor a more complete picture of any health concerns and how to treat them.<\/p><p>\u00a0<\/p><p>If a patient\u2019s doctor refers them to a specialist or there is an emergency, an individual\u2019s health record is already in the system.\u00a0 That means no more second-guessing the names of medications and no more duplicate lab orders.<\/p><p>The\u00a0health information will only be accessed on a need-to-know basis and providers determine what information will be included in a person\u2019s\u00a0Community Health Record that may include:<\/p><ul><li>Diagnoses and treatment<\/li><li>Allergies and drug reactions<\/li><li>Medications and immunizations<\/li><li>Lab and x-ray results<\/li><li>Other clinical reports<\/li><\/ul><p>\u00a0<\/p><p>Limited information may be used for community health purposes to identify health trends and access service needs. However,\u00a0a person\u00a0will never be personally identified for community health purposes.\u00a0 The PHIX team will only have access to\u00a0information to provide technical and administrative support.\u00a0 PHIX will not sell\u00a0a patient\u2019s\u00a0personal information for any reason.<\/p><p>\u00a0<\/p><p><strong>An individual does not have to participate.<\/strong> At Emergence Health Network, a client must \u201copt-in\u201d in order to participate and can only do so by completing an \u201copt-in\u201d form that must be given to them by their assigned EHN caseworker.\u00a0 Once completing the \u201copt-in\u201d form, consumers must then return it to their assigned caseworker for processing.<\/p><p>If an individual decides later to \u201copt-out\u201d of the program, they can.\u00a0 Simply request an \u201copt-out\u201d form from the designated caseworker, fill it out and return it to the caseworker for processing.
